Factors Associated with Physical Restraints by Licensed Caregivers Working at Daycare Centers: Focusing on Levels of Human Rights Awareness, Perceptions of Physical Restraints, and Organizational Culture

As elder abuse in care facilities has recently increased, there is growing concern about potential human rights violations by caregivers. The use of physical restraints to ensure the safety of older residents raises questions about whether this practice excessively infringes on their physical freedom. Given the blurry line between physical restraint and abuse, which could violate the rights of older adults, this study aims to provide data to help develop protocols that minimize the use of physical restraints by examining the factors that influence caregivers' experiences with these practices. We used the secondary data from 2,484 day care centers in 2017, leading to the final analysis of 7,042 care workers, including licensed caregivers, social workers, nurses, and nurse aides. Using frequency analysis, cross-tabulation, analysis of variance, and logistic regression, we found significant differences among workers in how they perceive, witness, and apply physical restraints. Licensed caregivers exhibited the greatest concern regarding human rights violations associated with physical restraints, yet they also reported the highest rates of performing or witnessing such practices. Notably, nurse aides were the most likely to use physical restraints, and the likelihood of performing restraints increased when they observed other workers engaging in such actions. Additionally, the care workers who perceived older residents as highly dependent or believed that government support was insufficient were more likely to use physical restraints. However, recognizing physical restraints as a human rights violation did not significantly influence whether care workers engaged in that practice. These findings suggest the need for education and policies tailored to the characteristics of each worker to minimize the use of physical restraints.
