Reconceptualizing Elder Neglect in Institutional Care Settings: A Systematic Review

초록

The establishment of long-term care insurance programs has increased public concern about the quality of care provided by nursing homes. However, elder abuse in nursing homes has continued to rise over the past decade, with neglect being the most common type of abuse. Understanding neglect has also become a key issue in debates about inadequate care, violations of older adults’ rights to self-determination, and the neglect of basic protections. Therefore, this study systematically summarized the concept and measurement of neglect in nursing facilities based on domestic and international literature on the neglect of older adults in nursing facilities, examining its meaning in greater detail. Three databases(RISS, PubMed, & PsycInfo) were used to conduct a Keybert analysis of academic articles published within the last 10 years. The articles were screened using Wordcloud and evaluated based on PRISMA guidelines and the QualSyst tool. Finally, 94 articles were selected for analysis. The main finding was that the concept of neglect must be reformulated to reflect changing times. It should go beyond basic protections, such as the absence of person-centered care, inadequate care, environmental neglect, unintentional neglect due to ignorance, and communication neglect. It should also include the absence of emotional interaction and the violation of residents’ right to self-determination. A clear understanding of neglect is important for improving the quality of care in nursing homes. In the future, it will be necessary to develop tools to measure this expanded concept of neglect.
