As exosome-based therapies and diagnostic tools advance, ethical concerns, challenges, and the technology’s future must be addressed. Exosome presents potential in precision medicine, medication delivery, and illness detection, also bringing up issues of safety, privacy, and equity. The long-term impacts and hazards, such as possible immunogenic reactions or unforeseen implications of genetic change, should be carefully considered before employing exosomes for targeted treatments, such as delivering gene-editing drugs or bridging the blood-brain barrier. To guarantee their fair and responsible usage, ethical considerations pertaining to the origin of exosomes, especially from human tissues, as well as the requirement for informed permission, transparency, and equal access to these treatments must be prioritized. Additionally, there are still technical obstacles in the areas of standardization, large-scale exosome production, and targeting mechanism tuning for accurate drug delivery. As the field develops, interdisciplinary cooperation and regulatory framework integration will be essential to overcoming these challenges and enabling exosomes to reach their full potential in clinical practice. Exosome research and use are expected to advance over the next decade, changing not only medical interventions but also how ethical considerations affect their use in society. © 2026 Elsevier Inc.
